Checklist of the requirements for Licensing Application (14 days)

1. Application form for business premises licence ( Composite, Advertisement and Entertainment Forms)
* For entertainment applications as concerts, expos, carnivals, sports program or any related events, the application should be sent at least 60 days before the program starts.

2. One (1) copy of identification card or a copy of information particulars of passport

3. Two (2) identification card/licence sized photos

4. ” Certified copy of the Company Registration ( Form 24 and form 49 ) ” in accordance with the Company Registration Act OR

5. Copy of Form A (Registration of Business) or

    Form B (Registration of Change in Business Details) oru

    Form D (Business Registration Certificate) in accordance with the

    Business Registration Ordinance 1965

6. The building Certificate of Fitness of for Occupation or lease agreement and permission from building owner.

7. Copy of Fire Extinguisher Receipt @ Fire Department letter of recommendation

8. One (1) sketch plan of the premise floor.

9. Visual Sketch of the signboard

Checklist of the Requirements for Night/Day Market Licence Application /Sari

1. Submit the application form for night/day market licences

2. Copy of TY2 injection and copy of Food Operator Course Certificate

3. Copy of Identification card

4. Two (2) passport sized photos.

5. Copy of business registration ( SSM )

Checklist of the Requirements for Hawkers’ Licence Application

1. Submit the hawkers licence application form.

2. Copy of TY2 injection and copy of Food Operator Course Certificate

3. Copy of business location plan

4. Copy of vehicle’s Registration Card and photo of vehicle for application involving mobile hawkers

5. Copy of Identification card

6. Two (2) passport sized photos.

EATING PREMISE APPLICATION
1. Copy of medical check-up by a doctor and TY2 injection
2. copy of Food Operator Course Certificate- *including all workers
PREMISES FOR NURSERIES/KINDERGARTEN/ TASKA / TASKI APPLICATION
1. Copy of approval letter from the Welfare Department for Taska / Taski
2. Copy of approval letter from the Education Department to operate Kindergartens
3. Copy of approval letter for Planning Permission (KM) for Taska/Taski/Tadika

Attention: The submission of the application forms for immediate licences must be made before 3.00 pm every day. All required documents must be complete. If you fail to furnish the complete documents, our, we cannot accept the application. For your cooperation, we wish to express our heartiest thanks.

  • Enforcing laws being applied by the Council.
  • Taking towing actions against vehicles parked in non-stipulated spaces.
  • Accompanying operations, controlling securities together with the other departments in the Council.
  • Controlling and preventing any construction of new building structures in the Council’s area.
  • Operations and actions in liaison with other government agencies.
  • Investigate complaints which are relayed by the public.
  • Taking disposal actions for seized goods.
